Need help identifying a monitor

I need to order a cap kit, as well as a few other parts from Bob Robbert's, but I am having trouble identifying my monitor. Also, which cap kit/components should I order, IE basic, deluxe, etc...?

*UPDATE: the monitor has been identified as a Wells Gardner 4900
